Application for Residency

If you would like to learn more about how to bring a performing, visual or literary artist into your school or community organization for an Artist in Residency program, please fill out and submit the form below.  Once recieved, your application will be reviewed by Gabriel Burian-Mohr, Arts in Education Program Manager.  You will be contacted either by telephone or email to discuss fulfilling your programming needs.  Gabriel can also be contacted at (541) 440-1991 or via email gabriel@uvarts.com.

How will you continue arts activities after the artist has completed the residency? How will those activities be incorporated into the curriculum?

Where will you publicize the AIE project that will take place at your site? (Please be sure to credit Umpqua Valley Arts Association, Oregon Arts Commission and the National Endowment for the Arts for funding assistance.)
